OSHA investigating death of worker servicing a forklift at Menards store in Sycamore, Illinois

LOS ANGELES , March 30, 2012 () – OSHA has begun an investigation into an accident that killed a Menards worker in Sycamore, Illinois, on March 27, The Daily Chronicle reported March 29.

The employee, Evaristo Alvarado, 56, was apparently trapped in a forklift he was servicing in Menards' back storage lot and was crushed.

The investigation will center on possible health or safey hazards that might have contributed to the accident, said an OSHA spokesperson.
